New York Foundation for Senior Citizens is dedicated to helping New York's seniors enjoy healthier, safer, more productive and dignified lives in their own homes and communities and to help them avoid the need for premature institutionalization.
Case Management Program
NYFSC’s Case Management Program helps coordinate home care, home delivered meals and other services for physically and mentally frail residents in Manhattan Community Districts 1, 2, 5 and 6. Working to help older adults remain safe, healthy and independent within their own homes, Case Managers meet with older adults age 60 plus at home to determine their eligibility for services, and then arrange and monitor the delivery of services. There is no fee for the case management services. There is a suggested contribution per meal for home-delivered meals and EISEP home care may have a sliding scale fee which is determined by the client’s income and housing expenses.
